Medical Student taking blood pressure at the brachial artery
In humans, the cuff is normally placed smoothly and snugly around an upper arm, at roughly the same vertical height as the heart while the subject is seated with the arm supported. Other sites of placement depend on species, it may include the flipper or tail. It is essential that the correct size of cuff is selected for the patient. Too small a cuff results in too high a pressure, while too large a cuff results in too low a pressure. For clinical measurements it is usual to measure and record both arms in the initial consultation to determine if the pressure is significantly higher in one arm than the other. A difference of 10 mm Hg may be a sign of coarctation of the aorta. If the arms read differently, the higher reading arm would be used for later readings.[citation needed] The cuff is inflated until the artery is completely occluded.
Oscillometric devices are commonly referred as Automatic Digital Sphygmomanometers or Digital Sphygmomanometers. These devices use an electronic pressure sensor for measuring the blood pressure and the readings are given out digitally on a display.
These devices have inflatable cuffs like Mercury or Aneroid Sphygmomanometers and the cuff is attached to the electronic unit. However, the main difference is in the technique used for measuring the blood pressure.
Mercury or Aneroid Sphygmomanometers reports are based on the sounds produced by the blood flowing inside the arteries. Digital Sphygmomanometers evaluates and measures the oscillations of the arteries using pressure sensors.
As the cuff is inflated and then deflated later, oscillations occurs. These oscillations are processed using an algorithm to produce systolic and diastolic values that are digitally displayed on the device display.
It is interesting to note that the working principles behind the oscillometric devices were discovered even before Korotkoff’s technique came into existence. However, due to various factors oscillometric approach remained unpopular till recently.
Automatic Digital Sphygmomanometers are usually battery operated. Some models designed for field use have dual power sources like battery and solar cells. They come in two variants:
Full-automatic blood pressure monitors: These devices have an electric pump for inflating the cuff. The operation of the device is very easy and requires minimum inputs from the user.
Once the cuff is placed on the upper arm, the device can be switched on and the reports are produced automatically.
Semi-automatic blood pressure monitors: The user has to inflate the cuff manually by hand using the bulb like a conventional device. Once inflated, the device can then start deflating the cuff automatically and beyond this point the reading is produced in a similar way as an automatic device.
These devices consume less power and can be more suitable for field operations where resources might be limited.
Wrist blood pressure monitors: These are digital blood pressure monitors that work similar to upper arm blood pressure monitors. It can be used by individual who find arm based devices uncomfortable or painful. However, medical experts do not recommend these devices for everyone, due to the possibility of receiving false reading due to improper use. Blood pressure monitors are very sensitive to body position and special care should be taken during their use in order to get accurate readings.
How complicated is it? If you’re tech savvy, you may feel comfortable using a smartphone compatible blood pressure monitor. But if not, then you may be better off with a simpler product. Most feature-rich digital blood pressure monitors are complicated compared to simpler types of blood pressure monitoring machines.
Will the cuff fit? Upper arm style blood pressure monitors are usually– but not always– adjustable. If your arm is too big or too small for the cuff, you won’t be able to use it. Most manufacturers list the size of the cuff on the side of the box. If you’re buying via the internet, check the product’s Amazon page or the manufacturer’s website for size info.

The ADC E-Sphyg 9002 Digital Sphygmomanometer features a unique backlit LCD display to reproduce the appearance of a traditional mercury column. It can be easily read in low light conditions. The ADC E-Sphyg 2 9002 offers a second numeric LCD display showing pressure, pulse, and a host of other vital information. The E-Sphyg 2 features an automatic setting. The manufacturing PCB assembly automatic mode can be used with children, adult, and large adult size cuffs to automatically obtain systolic/diastolic pressure and pulse in about 1 minute. The ADC E-Sphyg 2 9002 includes a rechargeable battery and an AC adapter.

CONTEC08A is a desktop Electronic Sphygmomanometer,High-definition color LCD display，supply English/Chinese interface,Strong visibility.Fully automatic blood pressure measure,it can be a household sphygmomanometer which is manufacturing PCB assembly used for adult, pediatric and neonatal.The device has the function of parameter measure,display and record output,and adopt data review interface such as "data list", "trend chart", "big font",with SpO2 measure function(with optional SpO2 probe).It can be applied to the clinique,physical center.
Main Features

■When there are factors which affect the measure in the process of measure and the device can't get the measure result,the device can display the corresponding error message.
■With short-time power storage function, when replacing the battery, the clock time function can't be affected.
■Supply two kinds of NIBP measure unit: mmHg / kPa
■Store measure results with date and time.
Results
Evaluation of the accuracy of digital sphygmomanometers is a simple two-phase process: (1) comparison of the pressure sensors against a mercury column and (2) assessment of the ability to detect Korotkoff sounds. The only equipment needed is a mercury column, a Y tubing, a “dummy” arm, two male-female adapters, and a stethoscope. The two short ends of the Y tubing are used to connect the mercury manufacturing PCB assembly sphygmomanometer and the digital unit, and the long end of the Y tubing is attached to the cuff of the digital unit. The inflation mechanism of the digital unit (manual or automatic) influences the approach used for comparing the two units. For both manually and automatically inflatable digital units, the digital display should be within ±4 mm Hg of the mercury level; likewise, a properly functioning digital unit should detect Korotkoff sounds (both systolic and diastolic readings) within ±4 mm Hg of the auscultated blood pressure measurements. If the digital sphygmomanometer is judged to be accurate, 6-month reevaluations are suggested.
Introduction: The mercury sphygmomanometer has been the gold standard for blood pressure measurement, but it is not practical to be used in community setting. Aneroid and digital sphygmomanometers manufacturing PCB assembly portability are higher compared to mercury device, but the reliability are still in doubt. The purpose of this study is to compare blood pressure measurment by mercury, aneroid and digital sphygmomanometer.
Methods: The study was a cross sectional study conducted in April - October 2014 in Bandung. A total subject of 108 people from minimal sample size of 66 is obtained through non probability sampling technique. Blood pressure was measured on dominant arm at seated position, the measurement was conducted by using mercury, aneroid and digital sphygmomanometer, twice for each device. Blood pressure measurement was manufacturing PCB assembly conducted by following protocol from 'European Society of Hypertension International Protocol revision 2010 for the validation of blood pressure measuring devices in adults'. The data was analyzed by Paired Sample T-Test for comparing blood pressure measured by mercury, aneroid and digital sphygmomanometer with significance p value < 0.05.

Even the most advanced devices can produce incorrect reading with some individuals. Therefore, it necessary to periodically counter check with conventional Mercury sphygmomanometers for accuracy.
With a manual instrument, listening with a stethoscope to the brachial artery at the elbow, the examiner slowly releases the pressure in the cuff. As the pressure in the cuffs falls, a "whooshing" or pounding sound is heard (see Korotkoff sounds) when blood flow first starts again in the artery. The pressure at which this sound began is noted and recorded as the systolic blood pressure. The cuff pressure is further released until the sound can no longer be heard. This is recorded as the diastolic blood pressure. In noisy environments where auscultation is impossible (such as the scenes often encountered in emergency medicine), systolic blood pressure alone may be read by releasing the pressure until a radial pulse is palpated (felt). In veterinary medicine, auscultation is rarely of use, and palpation or visualization of pulse distal to the sphygmomanometer is used to detect systolic pressure.
Digital instruments use a cuff which may be placed, according to the instrument, around the upper arm, wrist, or a finger, in all cases elevated to the same height as the heart. They inflate the cuff and gradually reduce the pressure in the same way as a manual meter, and measure blood pressures by the oscillometric method.[4]

By observing the mercury in the column while releasing the air pressure with a control valve, one can read the values of the blood pressure in mm Hg. The peak pressure in the arteries during the cardiac cycle is the systolic pressure, and the lowest pressure (at the resting phase of the cardiac cycle) is the diastolic pressure. A stethoscope is used in the auscultatory method. Systolic pressure (first phase) is identified with the first of the continuous Korotkoff sounds. Diastolic pressure is identified at the moment the Korotkoff sounds disappear (fifth phase).
Measurement of the blood pressure is carried out in the diagnosis and treatment of hypertension (high blood pressure), and in many other healthcare scenarios.
